The aim of this study was to determine the clinical and laboratory values that define chorioamnionitis at term and its effect on maternal and neonatal outcomes. Prospective observational study. University tertiary medical center. Patients with term singletons with a diagnosis of preterm labor and intact membranes. None. Bacteriologic cultures of membranes and maternal blood, fetal arterial and venous blood, neonatal arterial blood, and cerebrospinal fluid (CSF) were performed. Patients were assigned to one of two groups based on the presence or absence of a definitive bacterial culture. Thirty-two patients were enrolled. Of the 32 patients, 6 had a definitive bacterial culture. Amniotic fluid was cultured in only one patient. No patients had any symptom of maternal fever or uterine tenderness. None of the patients had white blood cell counts above 15,000. Amniotic fluid analysis was performed in only one patient and had a leukocyte count of 21 cells per microliter. Only one patient had a positive culture of blood (maternal). This patient had a positive culture of amniotic fluid. Neonatal and maternal outcomes in both groups were not significantly different. Preterm labor with intact membranes without any symptoms of maternal fever or uterine tenderness is not an indicator of chorioamnionitis at term.Q:
